1. Use the http://drupal.org/project/pathauto module
2. Read up on http://drupal.org/node/117201 to know why using text is better than using flags, and you'll get a free pointer on the theming function to alter to remove the flags (or the text)
3.  
a) Using complete different menu for different language is definitely something you will need for sites that have a lot less content in one language compared to the other. Or if the order of the menu's is logically different in one language compared to the other, ...
b) Have a look at drupal 6 (currently beta 2) which has build in support for multilangual sites. It's not that all i18n functionality is build in, but it is cleaner and easier to use.
c) Will having drupal 6 installed, also have a look add the actions/triggers functionality which allows you to email an editor/translator when new content is posted, or bring a node (and it's out-of-sync translated counterpart) offline as soon as it is updated.  So that you can first bring the translations back in sync before publishing them again.
d) Be sure to check out http://drupal.be for more help in your favorite language
